1st pressing: Label states "true mono", back of Picture Sleeve states "For extra copies, send one dollar to...", Lyric sheet with "once more you morons" and "(Published by Lexitron Music/ASCAP)"
Some 2nd pressing vinyl (labels say "45 rpm" instead of "true mono") was distributed with 1st pressing sleeves/inserts
Black side (B) plays Forming and 'true mono' side (A) plays Sex Boy. Around 1000 pressed, around 800 destroyed (thrown off a cliff bc of the label error), leaving around 200 that were distributed.
The untitled live song (The Germs Live) is "Sex Boy", recorded at the second Germs live show, at the Roxy.
